Registered Nurse (RN) | Case Manager Location: Oklahoma City, OK Agency: Cynet Health Pay: $2,432 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 12 Weeks Start Date: 2/9/2026 About the Position Job Title: Lead Case Manager Profession: RN Specialty: Case Management Duration: 12 months Shift: Day Hours per Shift: 8:00 AM - 4:30 PM Experience: Minimum of 2 years social work or case management experience in an inpatient setting preferred; previous leadership or supervisory experience strongly preferred. License: Current Registered Nurse or Social Work licensure Certifications: Certification in Case Management or Rehabilitation preferred; for example, Commission for Case Manager Certification (CCM); Association of Rehabilitation Nurses (ARN) certification, American Case Management Association (ACM) or Board Certification in CM by the ANCC. Must-Have Skills/Experience: Basic computer knowledge. Effective communication skills in English (verbal and written). Experience in social work or case management in an inpatient setting is highly preferred. Acute or rehabilitation hospital experience is preferred. Description: This position involves assisting with discharge planning. The role includes conducting utilization reviews. Family conferences and team conferences will be part of the responsibilities. The case manager will address other required patient needs and resources necessary for safe discharges. The patient population includes individuals 18 years and older. Common diagnoses include stroke, traumatic brain injury, orthopedic conditions, cardiac issues, amputation, and neurological conditions.
