Job Description:
Vital Caring Group is seeking compassionate and dedicated
Hospice Aides to join our team. We focus on delivering
personalized, comprehensive home health solutions to enhance
patient outcomes. As a Hospice Aide, you will work directly with
patients in their homes, providing care that makes a significant
difference in their final days. This position requires a blend of
empathy, medical knowledge, and patient care skills to help manage
physical needs and comfort. Hospice Aides are crucial members of
our healthcare team, offering support and comfort to patients and
their families during challenging times. Provide daily care to
hospice patients including personal hygiene assistance, dressing,
and ambulation. Monitor patient’s health condition and report any
significant changes to the supervising nurse. Assist in the
administration of medications as directed by a physician or nurse.
Offer emotional support and compassionate care to patients and
their families. Maintain accurate records of patient care,
condition, and progress. Strong interpersonal and communication
skills Ability to provide physical assistance and care to patients
Knowledge of basic health care procedures Capability to work as
part of a team and follow nursing instructions Medical Insurance

Nursing Assistant (CNA) or equivalent certification CPR
certified
