Current Oklahoma Physical Therapy license required prior to the start of assignment. Current CPR certification required. Valid driver’s license, vehicle insurance, and access to a dependable vehicle for travel within the agency service area. Experience providing home health physical therapy preferred. Ability to complete evaluations, develop plans of care, and document per CMS and agency requirements within state timelines. Willingness to participate in weekend rotation and potential availability on observed holidays as needed. Must be able to visit patients across the agency’s service area and float to other nearby agency locations as needed. Assignment Details Primary Specialty: Physical Therapist.
