Job Description:
The RN Care Manager position serves Licking and Knox counties,
offering full-time hours (8:00A-4:30P) with weekend and holiday
rotations.
It involves planning and delivering patient care through
assessment, planning, interventions, and evaluation, collaborating
with team members, patients, and families.
Qualifications include RN licensure in Ohio, a minimum of two years
acute care experience preferred, CPR certification, and valid
driver--------s license with insurance. Certification in Hospice
and Palliative Medicine (CHPN) is preferred and supported.
